OSHA 300, OSHA 300A, & OSHA 301 2024

The Occupational Safety and Health Administration (OSHA) has several recordkeeping requirements for employers to meet on an annual basis, with three types of form requirements:


  • OSHA 300 – a log of work-related injuries and illnesses in the previous year.

  • OSHA 300A – a summary of work-related injuries and illnesses in the previous year.

  • OSHA 301 – Injury and Illness Incident Report


Please note, most employers with 11 or more employees must complete the recordkeeping requirements. There is an exemption for employers with 10 or fewer employees, and some industries are exempt. The OSHA 300A form, the summary of work-related injuries and illnesses from the previous year, must be posted at each work-site (establishment) on February 1 of each year, and must stay posted through April 30, of the year.


Some employers may also be required to submit their recordkeeping to OSHA electronically each year:

  • Employers with 20 – 249 employees in high-risk industries must submit the OSHA 300A form electronically each year, no later than March 2.

  • Employers with 250+ employees in high-risk industries must submit the OSHA 300A form electronically each year, no later than March 2.


Employers with fewer than 20 employees do not have to submit information electronically to OSHA.


As of January 1, 2024, employers with 100+ employees at any establishment at any time in the previous calendar year will be required to submit OSHA 300, 300A, and 301 forms each year if the establishment is in an OSHA-classified industry with elevated injury and illness rates. Employers must keep the OSHA forms listed above on file for a minimum of five (5) years. These forms do not need to be sent to OSHA directly, unless requested by OSHA, but should be available during the required posting period each year for all employees.
